Activities Office

Before any student is able participate they need to have the following forms filled out and on file in the Activities Office:

  • Behavior Contract
  • MSHSL Eligibility Brochure - if applicable
  • Insurance/Transportation Waiver
  • Sports Qualifying Physical (within the last 3 years) - if applicable
Once it is verified that the student has all forms filed, they will be issued an Emergency Card. That emergency card must be handed to the coach on the first day of practice. It is their " ticket to practice." Without the emergency card the student will not be allowed to practice -- NO EXCEPTIONS.

Athletic Department Forms

Every student will need to have all of these forms on file as well as an emergency card before they are able to participate in athletic programs at Totino-Grace High School.

These forms, with the exception of the physical form, will be mailed to each student during the summer. They will be collected at Eagle Round Up in August.
